  • Timeline: Dates align with the declared scenario timeline.
  • Context: Information supports the scenario narrative (training context).
  • Notes: Reviewer note explains what was checked and why.

How to write an escalation note (training)

Use this structure:

  • Observed: neutral facts (what you see)
  • Risk: why it matters in training context
  • Action: what you would do next (secondary review / clarification / record & close)

This is a training format that helps learners avoid vague notes like “looks suspicious” without explaining why.
